Attention – Applicants for the 2007 Scholarship Program

Due to technical difficulties, we can not guarantee that applications for 2007 were successfully uploaded.  Please contact Mike Valdez, ( afentvet@comcast.net ) and Joe Herre, (papajoedh@hotmail.com ) to confirm that all needed forms have been received.  For anyone who is applying, Please do as follows:

  1. E-mail your DD214 in either a .pdf, .jpeg, or .gif format to the following addresses; Mike Valdez, ( afentvet@comcast.net ) and Joe Herre, (papajoedh@hotmail.com )
  2. Click on the 2007 Application link in this Box to open the pdf file of the form.  Print this out and complete, then copy and paste this into either a Microsoft Word document or as a .pdf file and E-mail to the following addresses;  Mike Valdez, ( afentvet@comcast.net ) and Joe Herre, (papajoedh@hotmail.com )
  3. Click on the 2007 Program Director;s Form and print out this form. Have your Program Director fill out the Program Directors Form, then copy and paste this into a Microsoft Word document or as a .pdf file and E-mail to the following addresses;  Mike Valdez, ( afentvet@comcast.net ) and Joe Herre, (papajoedh@hotmail.com  )

2007 Scholarship Application

2007 Program Director's Recommendation

 

Part 1 - DD-214
Eligibility Criteria:
1. Any Honorably Discharged Veteran of any Uniformed Service of the United States, or active member of the Guard or Reserve of any Uniformed Service of the United States enrolled in Physician Assistant Training at an accredited PA program in the USA;
 
2. Sons or Daughters of any United States Army Special Forces members (must have SF Regiment designation) may compete for the Chan-Padgett Scholarship. (Applicants for this scholarship must include a scanned DD-214 for the US Army Special Forces member who was their sponsor). Please indicate your submission for this special scholarship in the comments section of the online application.
 
3. Sons or Daughters of any past or present United States Air Force members with honorable service may apply for the SSGT Robert V. Milner Memorial scholarship. (Applicants for this scholarship must include a scanned DD-214 for the US Air Force Forces member who was/is their sponsor). Please indicate your submission for this special scholarship in the comments section of the online application.

Application Process:
1.Electronic applications will be accepted after October 1st each year for the following yearly cycle.
2. Deadline for all application material to be submitted electronically is March 1st of each year.
3. Student and faculty advised of selection by May 1st of each year.
4. Scholarship checks mailed to PA Program Director for presentation by May 15th of each year.

Applicants for this scholarship must upload a scanned DD-214 for the US Army Special Forces member who was their sponsor. You cannot continue with the Scholarship Application unless this first step is done. Acceptable formats: .pdf, .jpg, .gif, .png .

Failure to provide this document will void this application. NO FAX DOCUMENTS CAN BE ACCEPTED. IT IS THE RESPONSIBILITY OF THE APPLICANT TO SUBMIT THE FORMS REQUESTED IN THE PROPER FORMAT.

APPLICATION DEADLINE IS 2400 Hours, MARCH 1ST, 2007. Applicants will be contacted by e-mail when a completed application is received.

Instructions for completing the Scholarship Application
1. Upload the DD-214 file.
2. Submit the rest of the online application. Suggestion: make a copy of your document before you hit the submit key.
3. Complete Student Section of the Director Recommendation form and email to your Program Director.
4. Request that Program Director complete Recommendation form and submit online.
